This article proposes an action guide to making decisions regarding the ethical allocation of resources that affect access to healthcare services offered by community-based healthcare organizations. Using the filter of empirical data from a study of decision making in two community-based healthcare organizations, we identify potentially relevant conceptual guidance from a review of frameworks and action guides in the public health, health policy, and organizational ethics literature. We describe the development of this action guide. We used data from a prior (...) empirical study of the values that influence decision making about the allocation of resources in particular types of community-based healthcare organizations. We evaluated, organized, and specified the conceptual guidance we found in 14 frameworks for ethical decision making. The result is an action guide that includes four domains that are relevant to the context of the decision to be made, eight domains that are relevant to the process of the decision to be made, and 15 domains that are relevant to the criteria of the decision to be made. We demonstrate the potential use of this action guide by walking through an illustrative resource allocation decision. The action guide provides community-based healthcare organizations with a conceptually grounded, empirically informed framework for ethical decision making. (shrink)

The Hoffman Report scandal demonstrates that ethics is not objective and ahistorical, contradicting the comforting progressive story about ethics many students receive. This modern-day ethical failure illustrates some of the weaknesses of the current ethics code: it is rule-based, emphasizes punishments for noncompliance, and assumes a rational actor who can make tricky ethical decisions using a cost–benefit analysis. This rational emphasis translates into pedagogy: the cure for unethical behavior is more education. Yet such an approach seems unlikely to foster ethical (...) behavior in the real world, either for students or for mature scientists. This article argues for an alternative ethical system and a different way of teaching ethical behavior. Virtue ethics emphasizes the development of ethical habits and traits through regular practice and reflection. We show how virtue ethics complements a feminist approach to science, in which scientists are encouraged to reflect on their own biases, rather than attempting to achieve an impossible objectivity. Our article concludes with pedagogical suggestions for teaching ethical behavior as a practical and intelligent skill. (shrink)

Miranda Fricker maintains that testimonial responsibility is the proper corrective to testimonial injustice. She proposes a perceptual-like “testimonial sensibility” to explain the transmission of knowledge through testimony. This sensibility is the means by which a hearer perceives an interlocutor's credibility level. When prejudice causes a hearer to inappropriately deflate the credibility attributed to a speaker, the sensibility may have functioned unreliably. Testimonial responsibility, she claims, will make the capacity reliable by reinflating credibility levels to their proper degree. I argue that (...) testimonial sensitivity may be or involve “mindreading,” the cognitive capacity by which we predict human behavior and explain it in terms of mental states. Further, I claim that, if testimonial sensibility is or involves mindreading, and mindreading is a function of brain processes, testimonial injustice cannot be corrected by testimonial responsibility. This is because 1) it appears to rely on conscious awareness of prejudice, whereas much bias occurs implicitly, and 2) it works at the individual level, whereas testimonial injustice occurs both individually and socially. I argue that the remedy for testimonial injustice is, instead, engaging in social efforts that work below the level of consciousness. (shrink)

Thomason argues that despite its obvious drawbacks and moral ambiguity, shame's place in our lives is essential.

Psychopathy refers to a range of complex behaviors and personality traits, including callousness and antisocial behavior, typically studied in criminal populations. Recent studies have used self-reports to examine psychopathic traits among noncriminal samples. The goal of the current study was to examine the underlying factor structure of the Self-Report of Psychopathy Scale–Short Form (SRP-SF) across complementary samples and examine the impact of gender on factor structure. We examined the structure of the SRP-SF among 2,554 young adults from three undergraduate samples (...) and a high-risk young adult sample. Using confirmatory factor analysis, a four-correlated factor model and a four-bifactor model showed good fit to the data. Evidence of weak invariance was found for both models across gender. These findings highlight that the SRP-SF is a useful measure of low-level psychopathic traits in noncriminal samples, although the underlying factor structure may not fully translate across men and women. What is an assurance? What do we do when we claim to know? Krista Lawlor offers an original account based on the work of J. L. Austin. She addresses challenges to contextualist semantic theories; resolves closure-based skeptical paradoxes; and helps us tread the line between acknowledging our fallibility and skepticism.

The persistence, and international expansion, of food banks as a non-governmental response to households experiencing food insecurity has been decried as an indicator of unacceptable levels of poverty in the countries in which they operate. In 1998, Poppendieck published a book, Sweet charity: emergency food and the end of entitlement, which has endured as an influential critique of food banks. Sweet charity‘s food bank critique is succinctly synthesized as encompassing seven deadly “ins” (1) inaccessibility, (2) inadequacy, (3) inappropriateness, (4) indignity, (...) (5) inefficiency, (6) insufficiency, and (7) instability. The purpose of this paper is to examine if and how the contemporary food bank critique differs from Sweet charity’s “ins” as a strategy for the formulation of synthesizing arguments for policy advocacy. We used critical interpretive synthesis methodology to identify relationships within and/or between existing critiques in the peer-reviewed literature as a means to create “‘synthetic constructs’ (new constructs generated through synthesis)” of circulating critiques. We analyzed 33 articles on food banks published since Sweet charity, with the “ins” as a starting point for coding. We found that the list of original “ins” related primarily to food bank operations has been consolidated over time. We found additional “ins” that extend the food bank critique beyond operations (ineffectiveness, inequality, institutionalization, invalidation of entitlements, invisibility). No synthetic construct emerged linking the critique of operational challenges facing food banks with one that suggests that food banks may be perpetuating inequity, posing a challenge for mutually supportive policy advocacy. In this paper, I compare Austin’s and Wittgenstein’s metaphilosophical positions, and show that they share many metaphilosophical commitments. Interestingly, we find Wittgenstein employing methods more frequently associated Austin and vice versa. Austin’s Sense and Sensibilia (1962) generates wildly different reactions among philosophers. Interpreting Austin on perception starts with a reading of this text, and this in turn requires reading into the lectures key ideas from Austin’s work on natural language and the theory of knowledge. The lectures paint a methodological agenda, and a sketch of some first-order philosophy, done the way Austin thinks it should be done. Crucially, Austin calls for philosophers to bring a deeper understanding of natural language meaning to (...) bear as they do their tasks. External economic aid has played an important role in Northern Ireland’s peacebuilding process, particularly by funding community-based intervention projects.As a consequence of the Troubles, Northern Ireland suffered from severe socioeconomic inequality. These locally funded projects have fostered social cohesion by encouraging cross community interaction aimed at reducing violence and sectarianism. The NGO projects also promote social justice, reduce inequality, and provide the means to meet people’s basic human needs. The field research for this article was conducted during the summer of (...) 2010 and explores the perceptions of 120 civil society leaders and funding agency development officers on the effectiveness of the aid from the IFI and EU Peace III Fund in creating local social-economic NGOs to promote equity, equality, and social justice. The findings of this study reveal significant diversity in the respondents’ descriptions of the aids’s role in promoting equality, equity, and social justice as well as their expressed hopes and frustrations regarding its overall effectiveness. The term food citizenship is defined as the practice of engaging in food-related behaviors that support, rather than threaten, the development of a democratic, socially and economically just, and environmentally sustainable food system. Ways to practice food citizenship are described and a role for universities in fostering food citizenship is suggested. Finally, four barriers to food citizenship are identified and described: the current food system, federal food and agriculture policy, local and institutional policies, and the culture of professional nutrition organizations.

Cloth, $69.95.This book is a valuable and thoroughly-researched contribution to the study of Plato's Socratic dialogues. Its fine qualities stem in part from its cathartic motivations: for years Beversluis suppressed his ever-growing reservations concerning (...) the "standard picture" of Socrates's interaction with his various interlocutors (16). On Beversluis's account, this position naively takes Socrates's own apologia at face-value, portraying him as a self-sacrificing soul-therapist who treats his corrupt, one-dimensional interlocutors fairly, despite their many carefully drawn faults (e.g., stupidity, vanity, and violence, followed by face-saving exits). Beversluis aims to rectify this picture by retrying Socrates in a class-action suit on behalf of Socrates's interlocutors—who, he thinks, the standard account has quite unfairly maligned (while it has simultaneously ignored Socrates's "dark side"; e.g., his unscrupulous, hypocritical, and harmful tactics [esp. his conscious use of fallacy]).Beversluis's book contains a Preface, Introduction, and seventeen chapters, finishing with a very good bibliography, general index, and indexes of passages cited, names, and modern authors. Chapter 1, "The Socratic Interlocutor," examines the structure of Plato's Socratic dialogues and the role of their interlocutors. Beversluis contrasts Plato's dialogues with those produced by Augustine, Anselm, Hume, and Berkeley, arguing that a key difference between them lies in the fact that Socrates is presented as having no system to expound, championing instead the intrinsic value of philosophizing. Also, Socrates's interlocutors are drawn from a "narrow sociological group," (29) and are unprepared for their (unpleasant) encounters with Socrates (31).Chapter 2, "Elenchus and sincere assent," cogently argues that the "say what you believe" constraint on Socrates's use of the elenchus represents a consensus in need of critical reappraisal. In particular, Beversluis contends that Socrates often relies on qualified or uncomprehending assent, and that our texts do not support the idea that Socrates never argues with the soul-ignoring intention to win an argument using faulty argumentation and willful misrepresentation of the interlocutor's statements.The next fourteen chapters closely assess Socrates's encounters with Crito, Ion, Hippias, Laches and Nicias, Charmides and Critias, Euthyphro, Cephalus, Polemarchus, Thrasymachus, Hippocrates, Protagoras, Gorgias, Polus, and Callicles. However, here readers are themselves given a mixed bag: while Beversluis offers, for example, a detailed and smart defense of Cephalus against those who portray him as a petty-minded, superstitious, moral fraud (Chapter 9)—as well as sensitive, careful analyses of Socrates's less-than-clear arguments throughout—other chapters occasionally go overboard in their apologetic efforts. One example will have to suffice here.According to Beversluis, Euthyphro's typical characterization as a pretentious, dense, father-bashing, religious zealot who "would have done well in the Nazi Youth Movement," (R. F. Holland; 163), has drawn attention away from the fact that Socrates's treatment of Euthyphro is abusive and ineffectual. But although Beversluis rightly sees that the figure of Euthyphro is being used in a complex fashion by Plato, his account is marred in several ways. For example, although it is likely that Socrates does not himself possess complete definitions of virtue-concepts such as justice (164-169), unlike Euthyphro, Socrates can presumably provide elenchus-proof defenses of his own non-traditionalist claims. Again, while Socrates may overwhelm Euthyphro with his principles of proper definition (Eu. 5c-d, 6d-e), proceeding on uncomprehending, merely verbal agreement (169-177), Plato surely did not intend... Abstract:This symposium includes thirteen personal narratives from people who have received at least one organ transplant from a living or deceased donor. These narratives foster better understanding of the experiences of life-saving organ recipients and their families, including post-transplant difficulties experienced—sometimes requiring multiple transplants. This issue also includes three commentaries by Macey L. Levan, Heather Lannon, and Vidya Fleetwood, Roslyn B. Mannon & Krista L. Lentine. Dr. Levan is a living kidney donor and associate professor of surgery and population (...) health. She writes and teaches on organ transplant and has expertise and experience in governance at the federal level as a member of the board of directors for the U.S. Organ Procurement and Transplantation Network. Dr. Lannon is a social worker and transplant advocate. Her work and interest in the topic were inspired by her husband who needed a heart transplant. Dr. Fleetwood is a surgeon specializing in liver transplantation, pancreas transplantation, kidney transplantation, as well as living donor nephrectomy. Dr. Mannon is a nephrologist and professor of medicine, pathology and microbiology. Her work focuses on understanding and improving long-term kidney transplant outcomes for patients, donors, and caregivers. Dr. Lentine is a nephrologist specializing in treatment of adult patients with kidney disease. WalshFrom childhood, David Slakter had undergone tests and invasive procedures to monitor his nephritis. It was not a surprise when in 2015, doctors told him he needed a kidney transplant. The wife of a childhood friend was a close match and gave him one of her kidneys. Before his transplant, aerobic exercise was difficult; a few months after transplant, (...) Slakter could ride his bike, hike, and work out with a personal trainer. He no longer had to weigh food or gauge his potassium consumption, a concern for people with kidney failure. For Slakter, “life post-transplant has been a significant improvement.”Individuals or family members of those who have received an organ transplant from a living or deceased donor share their stories in this issue of Narrative Inquiry in Bioethics. The symposium is titled “Receiving the Gift of Life: Stories from Organ Transplant Recipients.” Organ transplantation can extend an individual’s life by decades. It can also greatly improve an individual’s quality of life, as was Slakter’s experience. At the same time, organ transplantation is expensive and stressful. Recipients can experience complications, including organ rejection, and there are few appealing alternatives.After her heart transplant, author Leilani R. Graham required four open-heart surgeries, ECMO, and an intra-aortic balloon pump. She experienced delirium, necrotizing pneumonia, and severe muscular atrophy that required her to re-learn how to walk. She struggled with weight gain, explosive mood swings, hair loss, and was terrified that her body would reject the new heart. For Graham, “[Transplant] was not the miracle [she] was hoping for.” Graham says she felt unprepared for and inadequately informed about what to expect after her transplant, which left her “battered, beaten, and bruised” with a 12-inch line from neck to navel and “bullet holes” up and down her torso from the chest tubes. When reflecting on her experience post-transplant, Graham says, “It wasn’t the physical trauma that hurt the most. It was that no one seemed surprised [by the aftereffects of transplant].” Seemingly no one, that is except Graham, who calls on transplant providers to “prioritize empathy.”The symposium editors are Jason T. Pearlman. The relief of suffering is a primary goal of medicine. However, the authors note that clinicians often miss or ignore suffering because they focus on the causes rather than the experience of suffering, or the subjective nature of suffering makes it difficult to discuss. The authors believe that “the key to communicating about suffering lies in finding [End Page vii] the right kind of words to describe the experience” and say that figurative language offers a promising approach. In their research, the authors performed qualitative analysis on 52 works of literature and identified 254 excerpts of figurative language with themes of non-physical or psychological suffering. The authors found 13 salient themes: brokenness, diminishment, disorientation, drowning, emptiness, imprisonment, battle, darkness, isolation, invisibility, lifelessness, punishment, and torture. The authors reiterate that developing a shared language of suffering could foster a therapeutic patient-clinician relationship and improve clinicians’ ability to recognize and address a patient’s experience of suffering.María Susana Ciruzzi wrote the first of two case studies included in this issue. “The Sword of King Solomon” addresses a tragic and difficult subject—how to navigate the situation posed by conjoined twins when “any decision will produce damage and suffering.” The case involves two infant male patients born at 37 weeks gestation as omphalo-ischiopagus conjoined twins... Concerned with criticizing representational theories of knowledge by developing alternative concepts of knowing and communicating, Ian Angus and Lenore Langsdorf bring together eight essays that are united by a common theme: the convergence of philosophy and rhetoric. In the first chapter, Angus and Langsdorf illustrate the centrality of critical reasoning to the nature of questioning itself, arguing that human inquiry has entered a "new situation" where "the convictions and orientations that have traditionally marked the separation of rhetoric and philosophy—the concern (...) for truth and the focus on persuasion—have begun to converge on a new space that can be defined through the central term _discourse._"_ _In these essays, this convergence of rhetoric and philosophy is addressed as it presents itself to a variety of interests that transcend the traditional boundaries of these fields. Few emotions have divided opinion as deeply as shame. Some scholars have argued that shame is essentially a maladaptive emotion used to oppress minorities and reinforce stigmas and traumas, an emotion that leaves the self at the mercy of powerful others. Other scholars, however, have argued that the absence of a sense of shame in a subject--their shamelessness--is tantamount to a vicious moral insensitivity. As the twelve original chapters in this collection attest, however, shame scholars are entering a new phase, (...) one in which scholarship no longer attempts to defend one side of shame against the other, but rather accepts both faces as faithful to the phenomenon to be explained. At the core of our understanding of shame there are profound disagreements about the importance of the Other in shaping our moral identity. As this collection shows by its study of shame, the difficulty of the connection between Self, Other, and morality spans over millennia and cultures and currently animates important debates at the core of feminism and disability studies. The sports world has historically rejected the practice of tanking. I argue that this attitude is unwarranted. To do so, I introduce a concept called strategic suboptimal play (SSP), which is the practice of incurring the risk of a short-term competitive disadvantage to increase the chances of gaining a longer-term competitive advantage. Tanking is just an instance of SSP employed in higher-order games, i.e. games that are at least partially played by other games, like tournaments or seasons. Since SSP is (...) integral to every sport, there are currently no easy grounds for rejecting it outright. As such, the same is true of tanking. I will explore some possible reasons why tanking is wrongly thought objectionable, as well as some objections to and implications of my argument for competitive sports more broadly. (shrink)
